Important information

Information Sharing  

The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information will only be shared with others to provide further medical treatment eg. From hospital services, to aid in getting help from others such as Social Work or when we have a duty of care such as Child Protection. Anonymised data may be used by the Health Board and Government to help plan services. If you do not wish your data to be used in such a way please let us know.  

 

Rights and Responsibilities  

You will be treated with respect and as a partner in your care. This means you have responsibilities for your own and your dependants health. 

We will ensure patients have access to medical care between 8-6pm Monday to Friday. We will work in partnership with you in all aspects of your medical care. We will promote good health and aim to prevent disease, illness and injury.  

In return we would ask that you let us know if you intend to cancel an appointment or are running late.  

Please treat staff with courtesy and respect. Inform us of any changes to your circumstances eg. Address and phone number.  

 

Violence Statement  

The practice considers aggressive behaviour to be any personal abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures. In the rare event of a patient being violent or abusive to staff, patients or others in the surgery we may call the Police, and we may ask the Health Board to remove you from our list.

Suggestions and Complaints  

We make every effort to deliver the best possible service. If you have any suggestions or wish to make a complaint please contact the Practice Manager, by asking at reception, or by letter.  We will review all suggestions we receive. If you have a complaint our Practice Manager will contact you to arrange to discuss your complaint.
